Necesito un formulario html o php, para capturar la fecha, num de cuenta, empresa del num de cuenta elegida, beneficiario, importe, concepto y mandar a imprimir en un cheque fisico, con los datos de la fecha, beneficiario, importe e importe en letra. Y que los datos del formulario se guarde en una base de datos mysql.
