I am hiring a ui/ux designer, who will work with me to design a lead generation website.

If you also know how to code your design into html/css that is a plus but not required.

We'd like to receive our final design in a psd file, and will also employ you to code in html/css if you are able to do so.

We'd like to hire someone who has created websites that convert.  It'd also be a plus if you were good at writing copy.

We'd want a main page that has a shortform with a few fields in it, then another longform application page that will have up to 40-60 fields on it.  We will also need a subpage that we can just put content on like about us, etc.

Send me a proposal, and also please provide some examples of your work - especially if you have examples of other lead generations sites you have designed.
