Website creation/design with connection to a database

Deliverable:

A website that will able to generate & process an access code (one code per submission) that when submitted allows the visitor to submit their:

- First/Last Names
- Phone Number
- Email Address


The information should be directly linked to a database and the information should be able to be categorized into different groups within the database. There will be additional pages that just need the design, our team will fill out those pages later independently.

*Website needs to be extremely mobile friendly*

Looking for a freelancer that has great communication skills and is a hard-worker. Also looking for a freelancer that will make edits and possibly do maintenance work moving forward after completion.
