Se solicita una pagina web con que permita registra datos de un servicio (código, Provincia, ciudad, Distrito, Localidad, código de servicio, código de supervisor, etc...)

Esta pagina deberá de tener una pagina de Usuario y contraseña, pagina de registro de datos y una pagina de dar de alta a nuevos usuarios, en donde los datos mínimos son usuario, contraseña, nombres Apellidos, Documento de Identidad.

ya tengo la BASE de DATOSEN MySql, solo son dos Tablas (USUARIOS Y DETALLE_SERVICIO)
