Formulário html/javascript, onde o usuário preenche os campos: nome, email, telefone, seleciona o estado em uma lista, seleciona uma cidade também de uma lista exibida de acordo com o estado, tem a possibilidade de anexar um arquivo ou não, e preenche a mensagem.

O ajuste é: fazer funcionar a função anexo e fazer com que a mensagem chegue na caixa de entrada do cliente.
