Crud simples - controle de contatos/matriculas

preciso de um crud para controle de pessoas que fazem contatos com nossa escola.
Esse crud teria as seguintes funções:

1) Cadastro dos Dados contatos. (Codinterno, nome, email, tel, ...Outros dados básicos)

1.1) Acompanhamento do contato
  Dia do contatdo, assunto, curso de interesse

1.1) Controle de resultado - se matriculou ou nao
    data - Nome - curso - valor - agente ..outros

4) relatórios
