Preciso desenvolver em java script um modal para que o cliente acesse o chat da empresa e insira seus dados básicos(imagem 1).

Ao clicar em enviar os dados sejam enviado para a imagem 2(customizada) e chamará a API da motion após receber as informações do cliente informado na imagem 1.
