Se tienen los formularios hechos en HTML5 y lo que se requiere es que los datos que se solicitan en el formulario, se guarden en una BD MySql para luego hacer las consultas respectivas o bien que dichos datos se exporten a Excel. Son alrededor de 6 formularios con un aproximado de 30 a 40 preguntas cada uno, todas de tipo selección, son muy pocas o contadas las opciones que requieren que se ingresen datos.
