Looking for a developer to turn a spreadsheet of calculations into a webform calculator

I'm looking to have a webform developed that calculates the opportunity cost of churn and referral improvements for SaaS and eCommerce subscription businesses.

The webform has to be integrated with a WordPress website (afterhoc.com).

There are 3 core elements that need to be built in two phases:
1. Phase 1 - core functionality: webform that calculates equations - i have a spreadsheet with the equations already that needs to be translated to a webform.
2. Phase 2 - backend system: user authentication - they tick a box for the privacy policy and it triggers a system email to give them access and basic information about how to use the tool. Ultimately, this will live behind a paywall. Save data to the account - can create new scenarios and load saved ones.

3. Nice to have (phase 2): export pdfs with a particular scenario and can add a title to the report.

I have an image of what I want the basic interface to look like. Everything in yellow is calculated and everything in green is manual entry. Once someone is hired for this project, I will supply a spreadsheet with all the calculations that will run behind the webform.

I want the webform to live at (afterhoc.com/churn-referral-calculator) and, after phase 2 of the project, only be accessible by logged-in/authenticated users (afterhoc.com/login).

It should be built so that in the future, it's possible to insert data (via API) into the yellow fields (Stripe, PayPal, Shopify, ThriveCart).

In terms of the tech stack, I would prefer the backend authentication and database systems to use Google Firebase.

I would prefer the developer to adopt a milestone-based approach to feature development.

I'm expecting a turnaround in a week or less for phase 1 of the project.
