Desarrollar un HTML (no responsive) con la línea gráfica aprobada.-

En concreto necesito pasar a HTML esta Landing: http://convenios.indisa.cl/escolar, para que el cliente lo pueda aplicar en su sitio, en esta página: http://www.indisa.cl/indisaweb/convenios/convenio-seguro-escolar-indisa.html?bt=convescolar.menu

Especificaciones:
-Dimensiones de la página del sitio (970)
-No incorporar header ni footer
-Considerar que el Formulario es un I-FRAME que permanece y además se extiende (se pueden agregar 4 beneficiarios hacia abajo) por lo que el background no se debe montar sobre el formulario.
