Hi, I would like to create a customization form for the purpose of "buy for me" in shopify platform.

I need to create a "shop for me" or "buy for me" form, the 5 requirement as following
1.  Allow customer to upload their item list with name , url, file upload feature. 
2.  Have conditional logic function that allow customer to add "new items" , and repeat the step 1. Customer also need to have the flexibility to adjust,delete ,update the items details before they submit the form.
3.  Once customer completed the order form, the data saved under "order" in shopify, to allow admin to check, and fullfill.
4. As since the data under "order", customer can view their "order status" everytime they login their account.
5. The form need to appear at 2 different pages.


We also need to link our shopify order form to google sheet at later stage.
