Online Assessment Centre for Recruitment

I need a simple web page that can be part of a recruitment assessment centre.
Upon opening the website, the candidate needs to select the language he or she is most fluent in (English, Cantoneses or Mandarin) The next steps must be available in all three language depending on the language chosen. However final results always a combo of Chosen language + English
1.
Fill up application form >> Fields will be full information of the candidate.
Two unique fields, [IC / Passport No] & [email address]
Ensure there is a verification page for the candidate to check the details before clicking submit.
2. Stage 1 questions...
If Fail say thank you for participating, please ask the recruiter to come to you.
Else, the form automatically proceeds to the next page.
4. Stage 2 question
If Fail say thank you for participating, please ask the recruiter to come to you.
Else, the form automatically proceeds to the next page.
3 stage 3 question
If Fail say thank you for participating, please ask the recruiter to come to you.

Else,
"Thank you for passing all stages. Please list the passing marks for all three stages"

Rules
Can only go to the previous page once
Candidate can only sit for the assessment a max of two times.
For the second round, if the email and/or IC is the same, no need to re-collect data.
Questions will refresh to show different set of questions and may not be the same as the first round.
Correct answers will never be displayed.
Only total correct answers.
Can do daily report to see how many fail at which stage
This website needs to be mobile and desktop friends.
Needs a reporting feature.
Needs an admin panel to upload new questions and print full answers.
Design simple with foodpanda letter head.
