I need a website that correlate with clinic workflow. It's clinic for HIV and sexually transmitted diseases Prevention/Test/Treatment

This is the workflow
Online booking > patient registration > self-questionnaire > laboratory investigation > examination > treatment > follow up > follow up reminder
1. Online booking
- Accessible by all staffs like a central slots for reservation
- can be summarized into hr of the day, day, week, month, year to see the market trend
- Automatically send email reminder for those who register with email

Patient registration
- collect patient data and generate set of number in a trackable way to represent patient anonymously
- all parameters can be searched and counted for statistical purpose
- customizable for further business expansion
- then must be printed out to be an actual paper for customer to hand sign, for legal purpose, scanned back and stored in a secure cloud
3. Self-questionnaire
- we think of this to avoid asking very personal matters, about sexual health, condom use, addictive drug use, frequency of sex
- So this is the only area of the website that a customer can see, it has to be linked very precisely with their patient number, has to have user-friendly look, and customizable due to rapid change of medical world nowadays
4. Lab investigation
- generate barcode from patient number for medical technician, has timing function to report when the test is finished, can be printed out, always stored in the database

Examination
Recorded by doctor, form has to be customizable, very confidential area, autosign by each doctor, able to be edited with information about who/when/why edit the data
6. Treatment
Done by nurse and doctor, I imagine this section to be like a checklist, has to be done completely, otherwise the system will be alarmed and the case can’t be proceeded to cashier
7. Follow up
Follow up section is a print out paper that auto calculate the date and time for patient for each specific treatment, customizable

Follow up reminder
- link to online booking, auto-send email reminder to provided emails. Template of email can be modified by staffs.
9. Cashier
- track every activities of each visit of the patient and calculate the cost for each visit, has discount function
10. Stock
Track the consumable products used each day and alarm when the stock is low, the alarming rate can be adjust too
11. Stats
Report numbers and any parameters that previously preset as keyword into graph/infographic that easy to comprehend.
