Se debe realizar una app usando ionic o phonegap, que debe conectarse a un webservice, mostrar datos del mismo, y podes scanear un código qr para la conexión.
La app es bastante simple.

1. Ingresa el nombre y numero de cel, Scanea un QR.
2. Envía el id y muestra info de un webservice.
3. Genera una notificacion en base a la info del web service.

Y eso es todo.
