Build Php/Mysql website with service checkout for Isp

We have screen layouts in FIGMA and need built an website with the following basic structure:
- 6 different pages:
1) *Landing  page* collects zip code and street number from visitor to check for fiber optic feasibility in that location
. Using google  maps geocoding api to find coordinates for a location provided by the visitor, check local database for available nearest access point and redirect user to main page or 'no service' page
2) *no service* simple page that states that there is no coverage and offers an form that user can fill with personal data for later contact. Record information on database.
3) *main page*: basic page with header and footer that shows 3 different highlighted service options from database
4) *shop page*: lists all offers with shopping cart displayed at the right vertical box, allows user to select one item per category of product (4 different available categories). Services listed show only 'preferred' offers from database on each category, and if user chooses to see all , we list them vertically.
5) *subscribe wizard*: 2 form pages with a total of 21 fields plus selected items from previous page, ending in a confirmation page. Data is to be recorded on database.
6) *self service page*: links to external systems for people who clicks in 'i am already a customer' link.
