Contents and scheduling

Hi there,
I need an appointment and scheduling software (definitely Windows but both apple and Windows preferable).
The software should be able to run from a single specific folder on my computer and should be able to create the exact same fields to each appointment that I would make.
The software should make folders for each file and store it in the parent folder in my computer.
Given, I was hoping for cloud based services integration, there should be a viewer facility to view it online. I should be able to add/edit online as well should there be a need.
The software should be able to generate the receipt or take payments and send invoices to PayPal and emails or fax numbers. I have the receipt/invoices items in excel format with their associated costs/charges, so the software should be able to import the excel file and should be able to update it (albeit manually) when there will be a new excel file available in next few years.
The software should be able to store information with the ability to import/export document as well in Pdf form. The payments balance sheet export should be in CSV as well along with the Pdf.
It should be customisable in a way that I can add more fields when creating appointments and able to create more folders in it in future should there be a need.
Most importantly, it should be able to generate reports based on date/customer/payment status (paid/unpaid/partly paid/invoiced/invoice pending)/work type/item number (from invoices). Suffice to say that it is important and I should be able to create more report types in future.
