Diseñar un formulario de contacto en una app en Kotlin

Tengo una app que estoy desarrollando en Kotlin mientras voy aprendiendo. Ya realizé el splashanimation, onboarding, formulario de inicio y registro con Firebase. Pero tengo un pequeño inconveniente, necesito ahora realizar un formulario de contacto, donde el usuario coloca en el formulario sus datos como: ubicación, fecha y hora, nro de telefono, y 2 botones de radio "Masculino" "Femenino".
Al realizar click en fecha se utilize (DatePicker) y hora (TimePicker). Y cuando se haga click en el boton ENVIAR. Se realice la acción1 o acción2:

acción1.
Se envien todos esos datos insertados por correo con (intent.ACTION_SEND)

acción2. Al momento de hacer click en el boton ENVIAR, los datos de vayan a una base de datos, y asi poder visualizar el pedido de servicio.
