Preciso criar um sistema php com mysql onde:

roda em servidores linux cpanel versão 7.3 php
O sistema será para cadastro SIMPLES de nome, número telefone e endereço (com mascara ddd + numero)
O sistema precisa identificar se o número já existe no banco de dados, se existir não gravar por cima, e sim retornar que já existe esse número.
