Mobile App Developer Needed for Health Tracking App

I'm looking for an iOS- and Android-compatible mobile application with:

- unique user login
- daily push notifications
- capability for the user to enter data in the form of both multiple choice and short answer question responses
- capability for the data entered into those responses to be tracked, by the user, over time with visual charts
- capability for the data of users to be made anonymous and viewed in aggregate by the app owner to understand what app features are being most used

I foresee the app having a total of 3-4 unique pages, with a simple user experience that is easy to navigate and visually appealing. The app does not need to integrate with other applications (such as built-in social sharing capabilities or smart watch connectivity). The app does not need to be able to host ads.
Users do not need to be able to communicate with each other through the app.
